The transition years are a time of great anticipation as youth and families plan for future adult life. It is a time when new agencies are important to connect with and new information is important to consider. At the same time, these new partners and resources can be confusing. The Agency Navigation Tool was created as a resource to provide basic information and about key agencies that may be involved at the time of a youth’s transition to adulthood. This tool is useful for youth, their families, schools, and agencies.
The brief video provided on this website introduces the Agency Navigation Tool, how it is structured, and the features provided in the tool. After viewing the video, download the Agency Navigation Tool and use this resource to begin exploring partner agencies in your local area.
