The Holmes MAP Team partnered with the local agencies serving transition students and used a root cause analysis to identify the barriers to student’s success in Holmes County. Their focus on building more employment opportunities and providers serving transition youth has led to more connections for students with partner agencies and receiving more services. Their list of accomplishments is growing and growing with a transition website specific to Holmes County and much more.
